Eligibility
If you are hired for a full-time position working 30 or more hours per week, you are eligible to enroll in the benefits described in this guide on the first of the month following 30 days of service. Per ACA regulations, variable hour employees will be measured over 11 months. If you work 30 or more hours per week during that time period, you will then meet the eligibility requirements to enroll in the benefits described.
Medical Health Savings Account
Choice between Allied, and Blue Cross Blue Shield. Through the High Deductible Health Plan, Blue Cross Blue Shield offers a Health Savings Account through Health Equity. Headway covers the cost of the banking fees for this benefit.
Dental
Coverage through Blue Cross Blue Shield. Covers preventative services, 80% of Basic services, 50% of major services, and 50% orthodontic care (children up to age 18) – $1500 lifetime maximum (In network).
Vision
Coverage through Blue Cross Blue Shield.
Voluntary Life / AD&D Guardian
Coverage for employee, spouse, and children.
Colonial
Employee’s choice to enroll. Coverage includes voluntary group accident, voluntary critical illness, short-term disability, medical bridge. Available on start date of employement
Employee Assistance Program (EAP)
EAP provide employees support services and health information for their health concerns, personal or family issues or work-related challenges. Available through the Hartford.
Schwab 401k Plan
Employees will be contacted by Schwab with enrollment details. Eligible on the first day of the next quarter following three full months of employment.
